Mission: It is the mission of Home Care of the Grand Valley, a not-for-profit agency, to be an integral home care link in the continuum of health services provided in a high quality, service oriented, and cost effective manner.

 

Vision: Home Care of the Grand Valley will provide the community with preeminent home and community based health services.

History

Home Care of the Grand Valley was developed as a non-profit, free-standing agency when St. Mary’s Home Care closed. Home Care of the Grand Valley was designed to serve the clients in need of highly skilled care. Since our inception, Home Care of the Grand Valley has served several thousand patients, clients and families in Mesa County and we have had the support and donations from organizations such as St. Mary's Hospital, Rocky Mountain Health Plans, Hilltop Community Resources over the years.

Because of our strong staff expertise and leadership, we have received several awards for quality and excellence in home health care service.

Our clinicians have a long history of collaboration with area physicians to help their clients get well after premature birth, complicated orthopedic surgeries and other illnesses. We value education and staying on the cutting edge of procedures and techniques to support our clinicians to be aware and responsive to your health care needs.

We are a locally-owned, non-profit agency founded in 2002, providing professional, highly skilled, in-home medical care and support services to Mesa County residents.

Our services and reputation have grown, but our commitment to a healthy community remains unchanged. As a non-profit agency, we feel compelled to support our community’s health care needs, even for those who have no ability to pay.

Our highly dedicated professionals offer excellence in therapy rehabilitation, newborn and pediatric care, wound care, nursing, social services, medical monitoring, personal hygiene and homemaking services. Personal Safety Alert (PSA) systems provide 24/7 help in the event of an emergency situation, such as a fall at home.

Our quality care is nationally recognized, with HomeCare Elite recently naming Home Care of the Grand Valley one of the top home health agencies in the country for the tenth year in a row.
