All nurses and therapists are provided at the direction of an established physician's plan of care for those patients who require nursing, home health aide and therapy services. Our care providers are available seven days per week as needed to meet the individual’s specific health care needs.

We are the Pediatrician’s choice for in-home services for newborns and children up to 18. We have a dedicated pediatric staff providing health care services to technology-supported children in the home. We function as an extension of hospital care when high-risk, intensive-care newborns are ready to go home.

Need help after an injury or fall? Our physical therapists are experts at working with you to improve your physical activity and mobility. We understand how important it is to you to return to normal daily activity following a hospital stay, fall or illness.

Physical Therapy develops, maintains and restores maximum movement and functional ability. This includes providing services in circumstances where movement and function are threatened by aging, injury, disease or environmental factors.
Occupational Therapy enables people with limitations or impairments to participate in everyday activities which have become difficult or impossible to accomplish. Occupational therapists work with individuals and families teaching new methods to make these activities possible again.
Speech Therapy assists with the rehabilitation of a person's speech, language, comprehension, voice, communication and swallowing issues. These problems may arise following a stroke or develop during chronic illness such as Parkinson’s.
Home Care of the Grand Valley is known for our highly skilled and knowledgeable clinicians and support staff. We provide the specialty care rehabilitation services that help those recovering from surgeries, accidents and illnesses, enabling them to return to work, if possible, and care for themselves.
